Liability Waiver Agreement
By submitting this waiver, the undersigned owner ("Owner") acknowledges and agrees that Fetchbell and its contracted dog walkers ("Service Provider") shall not be liable for any injury, illness, accident, or loss suffered by the dog named herein during scheduled walking services. The Owner represents that the named dog is in good health, up to date on all vaccinations (including rabies), and temperamentally suitable for group walks. The Owner accepts full responsibility for any damages or injuries caused by their dog to third parties, other animals, or property during the provision of services. The Owner authorizes the Service Provider to seek emergency veterinary care on behalf of their dog in the event of a medical emergency, and agrees to reimburse any related costs. This waiver shall remain in effect for the duration of services and may be revoked in writing at any time.
